To complete your membership registration, please follow the steps below:
1. Fill out the membership form: You’ll find the form attached. Please make sure to complete all required fields accurately.
2. Submit your payment: An annual membership fee of $10 is required. You may send your payment via Interac or by cheque.
3. Receive your membership card: Once we receive your completed form and payment, you’ll receive your annual membership card, giving you access to all member benefits.

I, the undersigned, wish to become a member of the General Association of Renal Patients (A.G.I.R.). I am including with my application a cheque made payable to the General Association of Renal Patients.
